Products Description
What is Exenatide Acetate?
Exenatide is a synthetic product of the glucagon-like peptide-1 (G LP-1) analog exendin-4, which consists of 39 amino acids. The Asn residue in the structure is prone to deamidation at high pH and high temperature to form Asp; His and Trp residues are more susceptible to oxidation; peptide bonds formed by Asp are more susceptible to hydrolysis and cleavage than other peptide bonds; except G1y All other amino acid residues are susceptible to racemization under base catalysis; under alkaline conditions, the Thr residue can be degraded by β elimination. In addition, temperature and metal ions also have an effect on beta elimination.
Exenatide acetate is a synthetic peptide drug derived from exendin-4, a hormone isolated from the saliva of the Gila monster. It is a long-acting glucagon-like peptide-1 (GLP-1) receptor agonist, formulated as its acetate salt for clinical use.

Main Functions & Pharmacological Effects
Enhance insulin secretion: It binds to and activates GLP-1 receptors on pancreatic beta cells, stimulating glucose-dependent insulin release. It only works when blood glucose is elevated, so it carries a low risk of hypoglycemia.
Inhibit glucagon secretion: Suppresses the secretion of glucagon from pancreatic alpha cells, reducing hepatic glucose output and lowering fasting blood sugar.
Slow gastric emptying: Delays stomach emptying, which prolongs satiety, reduces food intake and helps control postprandial blood glucose.
Appetite suppression: Acts on the central nervous system to curb appetite, facilitating weight loss or weight management.
Improve beta-cell function: May protect and improve the function of pancreatic beta cells over time.
Applications of Exenatide Acetate
Type 2 Diabetes MellitusIt is the primary indication. Used as monotherapy or combination therapy (with metformin, sulfonylureas, thiazolidinediones, insulin, etc.) to improve glycemic control in adults with type 2 diabetes. It is applicable for patients whose blood sugar cannot be well controlled by diet, exercise and oral antidiabetic drugs alone.
Weight managementLong-acting formulations of exenatide acetate are also approved for chronic weight management in adults with obesity or overweight accompanied by weight-related comorbidities, combined with calorie-restricted diet and regular physical activity.
Administration & Formulations
It is administered via subcutaneous injection. Two main formulations are available:
Immediate-release exenatide: Injected twice daily before meals.
Extended-release exenatide: A long-acting preparation injected once weekly for better medication adherence.
Initial dose: 5 mcg injected subcutaneously twice daily, within 60 minutes before the morning and evening meals.
Maintenance dose: After at least 1 month of treatment, the dose can be increased to 10 mcg twice daily if tolerated.
Formulation: Available as pre-filled pens containing 5 mcg or 10 mcg solution for injection.
Common Adverse Reactions
The most frequent side effects are mild to moderate gastrointestinal reactions, including nausea, vomiting, diarrhea and constipation, which usually alleviate as the body adapts to the drug. Rare adverse effects include hypoglycemia (mostly when combined with other antidiabetic drugs) and injection site reactions.
Precautions
Storage Guidelines
Before use: Store in the refrigerator between 2°C to 8°C (36°F to 46°F) until the expiration date.
After opening: Can be stored at room temperature (2°C to 25°C / 36°F to 77°F) for up to 30 days. Do not freeze or expose to direct light/heat.
General Warnings
Pancreatitis: Seek immediate medical attention if experiencing severe abdominal pain, which could indicate pancreatitis.
Hypoglycemia: Risk increases when used with sulfonylureas or insulin. May require dose adjustment of concurrent medications.
Allergic reactions: Discontinue use and contact a healthcare provider if symptoms like rash, swelling, or difficulty breathing occur.
Administration Tips
Inject into the thigh, abdomen, or upper arm; rotate injection sites to avoid irritation.
Do not inject after a meal or skip doses if a meal is missed.
Always use a new needle for each injection and do not share pens with others.
How Exenatide Acetate Works in the Body
Exenatide acetate is a GLP-1 receptor agonist, and it exerts glucose-regulating effects in multiple ways, all dependent on blood glucose levels:
Boosts glucose-dependent insulin releaseIt activates GLP-1 receptors on pancreatic beta cells. It only stimulates insulin secretion when blood sugar is high, which greatly lowers the risk of low blood sugar.
Suppresses excessive glucagon secretionIt inhibits glucagon release from pancreatic alpha cells. Reduced glucagon cuts down glucose production by the liver, helping lower fasting blood glucose.
Slows gastric emptyingIt delays the emptying of food from the stomach. This eases the spike of blood sugar after meals and prolongs feelings of fullness.
Reduces appetiteBy acting on relevant areas in the brain, it curbs hunger and decreases overall food intake, which aids weight control.
Protects pancreatic beta cellsLong-term use helps preserve the function of pancreatic beta cells, maintaining the body's ability to produce insulin over time.
Drug Interactions of Exenatide Acetate
Yes, exenatide acetate has notable drug interactions, mainly related to antidiabetic agents and oral medications.
Other antidiabetic drugsCombined use with sulfonylureas or basal insulin raises the risk of hypoglycemia. Dose reduction of sulfonylureas or insulin is usually required. When used with metformin or thiazolidinediones, the hypoglycemia risk is relatively low.
Oral medications requiring rapid absorptionExenatide slows gastric emptying, which may delay the absorption and reduce the efficacy of some oral drugs. For drugs that need fast absorption (e.g., antibiotics, oral contraceptives), take them at least 1 hour before exenatide injection.
WarfarinIsolated cases of increased international normalized ratio (INR) have been reported during combined use. Regular monitoring of blood coagulation indicators is recommended.
DigoxinIt may slightly decrease digoxin concentration. Clinical monitoring of digoxin levels is advised when they are used together.
