Tropicamide Powder

Tropicamide is an anticholinergic drug. At room temperature, it is a white crystalline powder, odorless, slightly soluble in water, and readily soluble in ethanol, dilute hydrochloric acid, dilute sulfuric acid and chloroform. It can block the excitation of the iris sphincter and ciliary muscle caused by acetylcholine. Its 0.5% solution can cause pupil dilation. A 1% solution can cause ciliary muscle paralysis and dilated pupils. It is mainly used in clinical practice to treat eye drops for mydriasis and accommodative paralysis.

Tropicamide is a synthetic derivative of tropinic acid. It has a relatively low dissociation constant, good intraocular permeability and strong tissue diffusion ability, which may be the reason for its rapid initiation and short maintenance time. The mydriatic and accommodative paralytic effects of 0.5% and 1% solutions of this product reach their peak within 20 to 30 minutes after eye drops. Subsequently, its effect gradually decreases, and it regulates paralysis (residual) for 2 to 6 hours. Dilate the pupils (the remaining ones) for about 7 hours. Topicamide is a drug similar to atropine, so it can cause a sharp increase in intraocular pressure in angle-closure glaucoma and may also trigger undiagnosed angle-closure glaucoma. The intensity of the ciliary muscle regulatory paralysis effect of topiricamide eye drops is closely related to the dose density. The four concentrations of 0.25%, 0.5%, 0.75% and 1% all have regulatory paralysis effects. After eye drops, the maximum residual accommodative diopters were 0.25% to 3.17 diopters and 1% to 1.30 diopters respectively. For residual accommodation that can be maintained at 2.0 diopters or lower, 0.75% and 1% solutions can last for 40 minutes, while 0.5% solutions can last approximately 15 minutes. Drop a 1% solution into the eye and wait for 5 to 25 minutes before dropping it again. This can achieve a more satisfactory cycloplegic effect for about 20 to 30 minutes. After 2 to 6 hours, one can read books and newspapers, and the regulatory function returns to the pre-drip level within 6 hours.

Mydriasis: Blocks acetylcholine-mediated contraction of the iris sphincter muscle, causing pupil dilation. A 0.5% solution induces mild mydriasis, while a 1% solution achieves more pronounced dilation.

Cycloplegia: Inhibits ciliary muscle contraction, temporarily paralyzing accommodation (focusing ability). This effect is critical for accurate refractive testing.

Ophthalmic Diagnostics

Refractive Error Assessment: Used to induce cycloplegia during retinoscopy and autorefraction, especially in children and patients with accommodative spasm.

Fundus Examination: Facilitates visualization of the retina, optic nerve, and peripheral eye structures by dilating the pupil.

Therapeutic Uses

Uveitis Management: Helps relieve ciliary spasm and reduce pain in inflammatory eye conditions.

Preoperative Preparation: Induces mydriasis before intraocular surgeries (e.g., cataract extraction).

Research Applications

Investigated for potential neurological effects, such as reducing audiogenic seizures in animal models of Fragile X syndrome (Fmr1KO mice).

Rapid Onset & Short Duration: Mydriasis peaks within 20–30 minutes, with cycloplegia lasting 2–6 hours and residual dilation resolving within ~7 hours. This minimizes patient discomfort compared to longer-acting agents like atropine.

Good Ocular Penetration: Low dissociation constant enhances intraocular permeability, ensuring effective action at the iris and ciliary muscle.

Contraindications: Angle-closure glaucoma (risk of acute intraocular pressure elevation) and hypersensitivity to anticholinergics.

Adverse Effects: Transient stinging, blurred vision, photophobia, and rarely, systemic effects (dry mouth, flushing) in pediatric patients.
